Court Ruling Requires Insurer to Pay Cost of Residential Treatment for Eating Disorder

EDMONDS, Wash., Nov 29, 2011 (BUSINESS WIRE) -- In the wake of a federal court ruling this summer that a California woman's insurance company should pay for residential treatment that she received for anorexia nervosa, psychologist Dr. Gregory Jantz echoed the court's opinion that residential care can be medically necessary to treat severe eating disorders.

Dr. Jantz, a nationally renowned specialist in eating disorders such as anorexia and bulimia, said that round-the-clock care can be required for such disorders because treating them effectively involves constantly monitoring patients' food intake and retention.

"Even intensive outpatient programs for eating disorders don't allow the opportunities for observation and intervention that a residential setting affords to therapists, nutritionists and nurses," said Dr. Jantz, who founded and operates The Center for Counseling and Health Resources, a residential treatment center based in Edmonds, Wash. Dr. Jantz is also the author of many books, including the best-selling Hope, Help and Healing for Eating Disorders, and operates the eating disorders website Caring Online.

According to Jantz, the California case revolved around Jeanene Harlick, an anorexia sufferer who received lifesaving treatment several years ago at a residential facility in St. Louis. Harlick had battled anorexia for more than 20 years, and when her condition worsened in 2006 despite intensive outpatient therapy, she checked into the Castlewood Treatment Center. Staff at Castlewood had to use a feeding tube to sufficiently increase Harlick's caloric intake.

Jantz said that Harlick's insurance company, Blue Shield of California, refused to pay for her nine-month stay at Castlewood, likening it to assisted living care, which her policy did not cover. Harlick's lawyers argued that undergoing residential treatment for anorexia is more comparable to receiving care at a skilled nursing facility, something that was covered by her policy.

The case hinged on application of California's 1999 mental health parity law. Parity laws were passed by some states to require insurers to cover treatment of mental illnesses to the same extent that they cover medical care or surgical procedures for physical ailments and injuries. A federal mental health parity law was passed in 2008, and the Harlick ruling is expected to have repercussions throughout the health care and health insurance industries.

Using a Holistic Approach to Treat Clients with Complex Co-Occurring Disorders

Complex co-occurring disorders- three or more diagnosable addictions or disorders-occur regularly in clients, causing them continued struggle with finding a meaningful life. This article will discuss how to effectively treat those clients that come to our offices, hospitals or residential treatment centers with complex co-occurring disorders.

It is imperative for treatment professionals to understand how to holistically treat clients who have complex co-occurring disorders. These types of clients, for example, have been diagnosed with an eating disorder, substance dependence, major depression, impulse control (self harm behaviors) and post traumatic stress disorder and/or anxiety disorder. They have spent many years trying to live a life battling these mental disorders and addictive diseases. Not until the mid 1980s did programs begin using the terms "co-occurring disorders" and "dual diagnosis" while attempting to treat co-morbid mood, thought and addictive disorders. These more comprehensive approaches address the whole person from a biopsycho-spiritual approach (Stillman-Corbitt, 2009).

As a social work professional, it is essential to arm clients with key recovery tools, allowing them to have hope for the future and an understanding of how to manage these disorders and addictions. "The treatment field is now confronted with an equally daunting challenge: a skyrocketing degree of acuity and complexity seen in outpatient and inpatient settings alike. Individuals presenting for care with a simple history of alcohol dependence and depression are a rare find." (Stillman-Corbitt, 2009)

After over 25 years treating clients in a variety of settings, it was only in the last five years that there was a realization of the importance of treating clients with a holistic approach. Abstract Background: Previous studies have shown low rates of eating disorders in some developing countries. We set out to investigate the prevalence of bulimia in an all-female college population in north India and relate it to sociocentrism of the culture. Methods: A total of 504 students in an all-girls private college in an industrial town in north India completed the Hindi translation of the Bulimia Investigatory Test, Edinburgh (BITE). A random sample of 50 students, irrespective of their scores, were interviewed using the DSM-III-R interview for eating disorders; they were also asked about qualitative aspects of their relationship with the family and their own views of their identity. Results: There was no effect of age, social class, religion or height on the distribution of BITE scores. Three key factors emerged; these were related to the constructs of compulsive activ ity, impulsivity/sociocentric avoidance and associated attitudinal responses and depressive thinking with fea tures of helplessness and feelings of failure.Acculturation was not related to BITE scores. Virtually all interviewees had sociocentric views of theirselves. Conclusions: Sociocentrism and impulsivity account for a significant amount of the variance. The role of sociocentrism in inuencing patterns of eating deserves to be studied further.

I have been here for the past two weeks and have been profoundly moved by the Indian women here in Tenali, a small region of India here in Andarha Pradesh. I came here on a mission trip with a group of people to serve the Harvest India orphans, elderly, leapors and women rescued from sex trafficking.

A new study has found that eating disorders such as anorexia and bulimia are associated with fertility problems, unplanned pregnancies and negative attitudes to pregnancy.

A team at King's College London and UCL investigated a group of 11,088 pregnant women from the Avon area of the UK.

Anorexia takes hold in India

In a country where many go hungry, anorexia is unknown to most
Most people in India struggle to get enough to eat - one estimate is that 60% of India's women are clinically malnourished.

But psychiatrists in urban areas are reporting cases of anorexia nervosa, the so-called slimming disease that can cause sufferers to starve themselves to death.

Most people in India have still not heard of the condition but Delhi psychiatrist Sanjay Chugh says he has seen an explosion in anorexia cases over the past few years.

He says a typical case could be a 17-year-old girl of around five feet six (1.68m) weighing just 4st 10lbs (30kg) who is convinced she is overweight.

"You can actually see her bones sticking out of her body," Dr Chugh says.

LUCKNOW: An Assocham survey on teenage anorexia has thrown up results that suggest that nearly 25% teenagers in Lucknow follow a diet plan and are anorexic.

Yet we receive many visitors to our Eating Disorder Hope website from India. So, the Eating Disorder Hope India blog and Eating Disorder Hope India page has been launched.

We found precious few, loosely based stats(extrapolated-highly unreliable, particularly in 3rd world countries) indicating that of the estimated 1,065,070,607 people in India, it is estimated that 7,455,494 suffer from binge eating disorder. 1,065,070 are estimated to struggle with anorexia, and 5,857,888 are estimated to suffer from bulimia. If these numbers are even remotely close to the real picture, there is a big need for eating disorder resources and information in India.
